Home Visits
Home visits are for those whose medical condition means they are unable to come to the surgery.
To request a same day home visit, please ensure you ring the practice before 10:30 am on 01274 662233. This gives us enough notice to arrange for a doctor to see you.
Calling after 10:30 am means we may not be able to offer you a same day home visit. In these cases, we can offer you a home visit for another day.
For urgent visits, call 111.
Extended Access Appointments
Our remote extended access appointments allow you to book evening (after 18:30) and weekend appointments to better fit in with your availability. These appointments are held at your choice of three sites:

GP Online Services
As part of our ongoing commitment to providing our patients with the best care possible we are offering both SystmOnline and Evergreen Life online appointment management services to all patients at our practice.
SystemOnline
SystmOnline allows patients or someone acting on behalf of the patient to proactively manage their care. The online services available include the ability to view an online Electronic Medical Record as well as booking appointments and managing medication.
SystmOnline is available as an online service or via apps, providing patients with convenience and flexibility when managing their care.
1. Visit the SystmOnline login page
2. Click on the Request Access to Online Services at ‘Bradford Moor Practice’ button.
3. Fill in all of the required fields and click Submit.
4. To complete your registration you must visit us with valid proof of identity e.g. passport, driving license etc.
Evergreen Life
Using the Evergreen Life service, you can book your appointments and order your repeat prescriptions online using the Evergreen Life app or desktop version.
To get on board Evergreen Life, simply contact reception to ask for your online access code.
Carers
“A carer is someone who, without payment, provides help and support to a friend, neighbour or relative who could not manage otherwise because of frailty, illness or disability. They may even be juggling paid work with caring responsibilities at home.” – Carers Resource
Carers have a tough job – it can mean unrelenting pressure, little chance to relax and a lot of worry. The system is complicated, and the issue is hidden.
If you are a carer, either ring us on 01274 662233, or use the form below to let us know. Doing this means we can register you as a carer, ensuring you receive the extra support and assistance available to you.
Barnardo’s Bradford Young Carers Service is a partnership between Barnardo’s, Bradford CCG’s and the Local Authority and with children and works with young people aged 5-18 years.

We now offer a carers clinic (run by Carers Resource) once a month at the Practice. Please ring us on 01274 662233 if you would like an appointment. Next clinic is on the 6th December.
